Title: 4-H Essential Elements Assessment Tool

Arizona Cooperative Extension in conjunction with National 4-H Headquarters are asking for your help with pilot testing a new youth development program self-assessment tool. You have been selected because you are either currently or recently involved in developing, implementing or evaluating a Cooperative Extension program for young people.

If you decide to participate, your participation will include completion of an on-line survey. Your participation is strictly voluntary and your identity will not be known to anyone including the principal investigator. You may refuse to participate or discontinue participation at any time without penalty. There is no risk and no benefit to your participation. You’re welcome to contact me at any time with questions (karenht@ag.arizona.edu) about the survey. You may also contact the University of Arizona, Human Subjects Protection Program office (520-626-6721) with your questions about research participants’ rights. If you decide to fill out the survey, your feedback will give us invaluable information about how to best support 4-H professionals and volunteers in their work with young people.

Any and all information we receive will be kept strictly confidential and will only be seen by authorized members of our staff. Data gathered from the survey will be summarized in the aggregate, excluding all references to any individual responses. The aggregated results of our analysis will be shared with National 4-H Headquarters and others interested in working with young people. These results would be used to improve professional development opportunities available throughout the Cooperative Extension System.

Your completion of the survey indicates your permission for use of the data for research purposes. After completing the 43 question survey and reviewing the resources pages and fact sheets, please go to http://cals-cf.calsnet.arizona.edu/fcs/4h/cssSite/assessmentQuestions.cfm and complete the feedback form.  Both parts of the online survey will only take about 20 minutes of your time. You will not be compensated for your participation. Thank you for your participation.
